Care Coordinators/Lawrence, MA and Lynn, MA
Intensive Care Coordination is one of the new services developed as part of Massachusetts’ Children’s Health Initiative (CBHI) that is restructuring and revolutionizing children’s mental health services. Master’s and Bachelor’s level Case Managers (or clinicians) support families with children who have behavioral and /or mental health issues using the Wraparound model. Apply your strong understanding of children’s mental health and family issues, as well as your cultural sensitivity, to work collaboratively in the field with multidisciplinary teams to develop individualized, strength based plans of care. Licensure a plus. Competency in Spanish or other languages highly desirable.

Family Partners/Lawrence, MA and Lynn, MA
Family Partnership is one of the new services developed as part of Massachusetts’ Children’s Health Initiative (CBHI) that is restructuring and revolutionizing children’s mental health services. Family Partners are parents or guardians of children with behavioral and/or emotional difficulties who apply their own life experiences in order to help educate and support similar parents and their families. Family Partners work collaboratively with families and professionals to: find community resources, teach parenting skills, navigate special education and mental health agencies, advocate, and empower parents. The position requires individuals who are mature and responsible, have effective verbal and written communication skills, reliable transportation, enjoy helping others, and can work independently as well as part of a team. Competency in Spanish or other languages highly desirable for Lynn. Bilingual Spanish/English a must for Lawrence.

Therapeutic Mentors/Lynn, Lawrence, Gloucester, Salem - MA
Responsibilities include: Developing activities aimed at reaching designated goals; Performing outcome assessments and progress notes in a timely manner; Engaging youth and families in a mentor program; Building strong working relationships with referring agencies, community organizations, and professionals; Requires: Associates or Bachelors in a human service field and one year experience working with children/adolescent age youth or a High School Diploma or GED and two years relevant experience. Must be a minimum of 21 years old with reliable and safe means of transportation and proof of valid auto registration, driver’s license, and auto insurance and must exhibit excellent written and verbal communication skills, be willing to learn about youth’s mental health issues, be familiar with and sensitive to diverse cultures and ethnicities, and willing to work flexible hours including weekends and evenings as needed.

In Home Therapists, Salem, MA
Master’s Level clinicians with a degree in counseling psychology, social work, or marriage/family therapy with license preferred (LICSW, LMHC, LMFT in MA) to carry out responsibilities that include: Comprehensive home based behavioral assessment inclusive of CANS, risk management, safety, and treatment planning, Intensive Family Therapy, Identification of community resources and development of natural supports for youth and parent/guardian/caregiver, Skills training for youth and parent/caregiver, Coaching in support of decision making in both crisis and non-crisis situations, 24/7 phone crisis response, Member outreach and transportation, Documentation, and participation in discharge planning. Requires experience providing family therapy, working therapeutically with children and adolescents with serious mental health issues and/or behavioral disorders and their families and experience with strength based assessment and development of treatment plans based on clear clinical formulations and knowledge of collateral systems. Must also be capable of working collaboratively within a team structure.

Clinicians: Lynn, Lawrence, Gloucester, Salem – MA
This position is responsible for providing effective mental health intervention to assigned clients (individuals and families), coordinating with primary care physicians, referrers, insurance carriers or other agencies, completing and maintaining documentation and records while representing CFFS in a professional manner that adheres to CFFS policies and guidelines and State and Federal regulations and program requirements. Duties include: Meeting with clients to assess their needs and provide complete diagnosis, and determining manageable problem focus and treatment priorities in collaboration with clients. (Clients may include: children, adolescents, families, adults, and elders); Presenting timely treatment plans to multi disciplinary team for review and utilization management and implementing treatment directives developed at team meetings; Interacting effectively with each client and working with clients, and when appropriate, their support systems to produce behavioral, cognitive, and /or emotional changes that meet treatment goals and then evaluating client progress and revising treatment plans as needed; Completing necessary documentation within required time parameters for each client (for example: assessments, treatment plans, insurance authorizations, problem oriented notes and treatment summaries); Participating effectively in group supervision, monthly staff meetings, and in quality improvement initiatives. Clinicians must maintain current license status and, if applicable, obtain licensure as agreed upon hire, adapt to new developments through continuing education, and have basic computer skills.
